Health Innovation

The CHANGE (Canadian Health Advanced by Nutrition and Graded Exercise) Program integrates nutrition and exercise into primary care settings.  Dietitians work collaboratively with a patient’s family doctor to design a personalized program focusing around prevention and wellness.

The CHANGE program is rethinking health care by shifting from a diseases care model to prevention and promoting health, giving families and individuals the skills they need to live healthy.
